KPS Chart Note Requirements for Rehab Therapy |

| KPS has implemented changes in outpatient physical, speech, occupational, and massage therapy charting. KPS not allow any time based therapy codes which do not have the beginning and ending times recorded in the patient's medical record. This can be documented in two manners 1) Start time - end time of each code 2) length of the treatment to the minute could be recorded instead. |

| Molina Healthcare has limited procedure codes from 97001-97542 to 24 visits maximum without pre-authorization. Basic Health is an exemption with pre-authorization required and limited to 12 visits per calendar year |
