Medicare NPI Billing

Medicare is now requiring all claims be submitted using your NPI provider and NPI group number. Don't get stuck in payment delays, bill it right the first time around. Remember -CMS-1500 claim form Box 24K should contain your individual provider legacy pin number AND your individual provider NPI number. CMS-1500 claim form Box 33 should contain the GROUP NPI number and the GROUP legacy pin number. If you are billing by paper you MUST include 1G in box 17a in front of the referring provider UPIN number and 1C in box 24i.

Kit for the 2007 Physician Quality Reporting Initiative (PQRI) that is
designed to assist eligible professionals— DPMs are eligible—with
successful reporting. The Tool Kit consists of some existing
educational resources plus new measure-specific worksheets to walk the
user step-by-step through reporting for each measure.

Molina Healthcare has limited procedure codes from 97001-97542 to 24 visits maximum without pre-authorization. Basic Health is an exemption with pre-authorization required and limited to 12 visits per calendar year

KPS Chart Note Requirements for Rehab Therapy

KPS has implemented changes in outpatient physical, speech, occupational, and massage therapy charting. KPS not allow any time based therapy codes which do not have the beginning and ending times recorded in the patient's medical record. This can be documented in two manners 1) Start time - end time of each code 2) length of the treatment to the minute could be recorded instead.
